Bone is living tissue, and needs balanced activity to maintain health. Approximately 35% of bone is composed of connective tissue fibers, mixed in a substantial amount of water. | A Multidisciplinary Approach to Contraceptive Counseling Nursing professionals. In addressing the complex issues associated with contraceptive choices that fit the needs and preferences expressed by individual women, there is growing recognition that appropriately trained professionals from a range of disciplines must be involved in delivering care. The role that nursing personnel, particularly those with advanced training e.g., clinical nurse practitioners, certified nurse midwives ; , can play in providing effective contraceptive services has been documented in the literature.12-17 The substantial involvement of nurse practitioners in contraceptive service delivery, including counseling, is due, at least in part, to commitment of Title X i.e., the Federal program devoted solely to the provision of family planning and reproductive health care ; funding to the training of more than 5, 000 nurse practitioners in the years between 1972 and 2000.221 One result of this resource commitment is that nurse practitioners have become the primary providers of Title X-supported services, accounting for an estimated 80% of Title X services delivered each year.222 In fiscal year 2000, Title X support for training of nurse practitioners was phased out and replaced with a "clinical specialty training program."221 This change in funding support has raised concerns that the program now in place will not meet staffing needs of Title X clinics.222 An assessment of the program conducted in 2004 suggests that, in fact, the training may be falling short in meeting the needs of clinical service sites. Among the problems cited with the Clinical Specialty Training Program were: 1 ; limited hands-on training; 2 ; difficulty addressing sequencing and timing of training in a way that meets the needs of all Title X programs; 3 ; potential duplication of training already offered regionally or locally; 4 ; difficulty tailoring the training content to meet the needs of individual programs; and 5 ; inaccessibility to most clinicians due to geographic and staffing limitations.223 The importance of specialized training, such as that offered through the Title X-supported nurse practitioners program, is suggested by the literature. Many nurses have reported having no formal training to prepare them to deliver contraceptive services that include advising patients on contraception and safe sex options, emergency contraception, and referral to appropriate specialist services.17 Nursing personnel involved in contraceptive services require enhanced training that helps them guide women in formulating a personal plan for achieving contraceptive goals, rather than simply providing general contraceptive information without regard to the specific circumstances in a woman's life.13, 18 Physician Assistants. Other primary care personnel are also increasingly involved in contraceptive service delivery, particularly in public health settings in which physician services may be limited. Physician assistants PAs ; are used in many rural and inner city clinical settings that serve low-income and or geographically remote populations. Data reported by the American Academy of Physician Assistants estimated that, in 2005, there were 58, 665 PAs practicing in the United States, including 18, 447 in family practice; 4, 991 in general internal medicine; 6, 060 in an internal medicine subspecialty; 1, 476 in obstetrics gynecology; and the remainder in one of eight specialty areas or other settings.19 These health professionals provided contraceptive services to over 6.8 million patients, including over 4.5 million in family practice settings and nearly a million 944, 573 ; in obstetrics gynecology settings.19 The literature on the scope and.
Prevalence of HIV-1 drug resistance continues to rise, and transmission of drug-resistant HIV-1 is also a growing problem. The U.S. Dept. of Health and Human Services, as well as other international agencies, recommends drug resistance testing as part of the routine management of patients with HIV-1 infection. There are two commonly used approaches for HIV-1 drug resistance testing: genotyping and phenotyping. Genotyping assays are based on sequencing the relevant portions of the HIV1 genome to detect drug resistance mutations. Currently available assays are designed for analysis of HIV-1 protease and reverse transcriptase. FDA-approved assay systems, such the ViroSeq HIV-1 Genotyping System, are available for laboratories wishing to do in-house testing. Genotyping assays are faster and less expensive than phenotyping assays, and are more sensitive for detecting emerging resistance. However, interpretation of drug-resistance mutations may be complex, and expert guidance is often recommended. The Virtual Phenotype, available from Tibotec-VIRCO, provides a computer-driven genotype interpretation that is derived from an extensive genotype-phenotype correlative data-base. Phenotyping assays, are offered by commercial companies Tibotec-VIRCO, ViroLogic, and Viralliance ; . RESEARCH ON DEPRESSION Assessment Several screening and assessment scales developed in outpatient settings have been validated in nursing homes. In one study, across all subjects, the 30-item Geriatric Depression Scale GDS ; was found to have sensitivity and specificity of 63% and 83%, respectively.15 These values increased to 84% sensitivity and 91% specificity for residents with Mini-Mental State Examination MMSE ; scores of 15 or greater. In another study, the GDS 30 items ; and the Brief Carrol Depression Rating Scale were found to have the best criterion validity when used in residents with greater than 50% prorated MMSE scores, and the GDS 15 items ; and the Center for Epidemiologic StudiesFDepression scale were rated as having fair validity.16 The Beck Depression Inventory was found to be unfeasible for most study participants, possibly because of the combined effect of its length and four-point rating for each item. One study found that nursing home staff use of instruments such as the Cornell Scale for Depression in Dementia or the Hamilton Depression Rating Scale HAMD ; increased sensitivity of depression diagnoses by staff from approximately 32% to 50%.17 Implementation of the Minimum Data Set MDS ; has prompted several studies comparing the validity of the MDS with that of other routinely used research instruments. The six such studies located included more than 400 subjects in more than 20 nursing homes.1823 These studies found that the MDS, as routinely collected by nursing home staff, has limited criterion validity, with correlation coefficients ranging from 0.15 to 0.44. In a study of 85 subjects, 21 it was found that the MDS identified only 24% of residents with a GDS score greater than 10. The study also found that, with a variation of the original MDS in which the seven items of the Sad or Anxious Mood section were summed and completed by research staff who also completed HAMD and Cornell Scale ratings, the MDS scale scores were statistically significantly associated with treatment response status as determined by other research instruments. Similarly, the MDS-based depression rating scale created by another study, 22 consisting of seven items of the MDS, when completed by nursing home staff observing the research nurse complete a HAMD, was found to have sensitivity and specificity of 91% and 69%, respectively, in comparison with the criterion standard of psychiatric diagnosis. Some guideline and opinion articles recommend routine laboratory testing for anemia, thyroid disease, infection, or metabolic disturbances, but this review found no studies of the systematic physical or laboratory evaluation of depressed nursing home residents. In the laboratory, cue-reactivity studies have shown that active imagery of audio scripts describing smoking urge scenarios reliably increased selfreported tobacco craving compared with neutral imagery scripts. Although studies have demonstrated the validity of imagery procedures to elicit tobacco-craving responses in single sessions, few studies have examined the consistency and reliability of responding in the same individuals over multiple experimental sessions. In this study, male non-abstinent smokers n 15 ; were presented with a randomized series of imagery scripts that varied in the intensity of smoking urge content and gender of voice presentation. At each of five sessions spaced over several weeks mean 25 days ; , participants were exposed to six imagery trials two each of no-, low-, and high-intensity imagery scripts ; . After each trial, participants completed three subjective measures of tobacco craving and mood Tobacco Craving Questionnaire, visual analog scales, and mood form ; . Ratings of positive mood decreased, whereas ratings of craving and negative mood significantly increased as a function of smoking-urge intensity, which was consistent across the five sessions. Further, significant intraclass correlations indicated that craving and mood responses were highly reliable over the five sessions, as well as across two, three, and four sessions. These results have practical implications for examining individual differences in sensitivity to smoking cues and for studies involving repeated measurement of elicited craving over time.
