Cordarone X Intravenous may cause moderate and transient reduction in blood pressure, and circulatory collapse may be precipitated by too rapid administration or overdosage. Atropine has been successfully used in such patients presenting with bradycardia. Temporary hot flushes, sweating, nausea have also been reported with Cordzrone X Intravenous. Amiodarone has been reported to cause frequent and potentially serious toxicity. The incidence, variety and severity of the effects varies from study to study. Most of the adverse effects are also related to dosage and duration of amiodarone, concurrent use of other antiarrhythmic agents, severity of underlying disease state, and individual variation in pharmacokinetic profile of the drug. More Common Reactions Biochemical Abnormalities Abnormal liver function tests increased AST, ALT and alkaline phosphatase ; have been reported. Abnormal thyroid function tests see `PRECAUTIONS Interference with Clinical, Laboratory and Other Tests' ; . Cardiovascular Atypical Ventricular Tachycardia torsades de pointes ; . Amiodarone-induced atypical ventricular tachycardia has been described. Earlier reports describe combination therapy in which other drugs, or clinical situations, could have been implicated. However, in 2 patients, given disopyramide and amiodarone, on withdrawal of the amiodarone, the disopyramide did not induce atypical ventricular tachycardia. Bradycardia: significant sinus bradycardia has occasionally been reported. Cardiac Failure: exacerbation of cardiac failure has been reported rarely. Other: sinus arrest and intrahisian block have been reported. Dermatological Photosensitivity commonly occurs in patients on amiodarone therapy. This can usually be alleviated by the use of topical sunscreen and other protective measures. Less frequently bluish skin discolouration and slate grey facial pigmentation have been reported. These adverse effects are partially dependent on dose and duration of treatment. Erythema, facial flushing and hair loss have been reported. Skin rashes, usually non-specific, including exceptional cases of exfoliative dermatitis have been reported; the relationship with the drug has not been formally established. Gastrointestinal Nausea and more rarely vomiting, anorexia, constipation and dysgeusia have been reported. Endocrine Effects on the Thyroid Both hyper- and hypothyroidism have occurred during or soon after treatment with amiodarone. Simple monitoring of the usual biochemical tests is confusing because some PBI and 131I uptake ; are invalidated and others T4, T3 and FTI ; may be altered where the patient is clearly euthyroid. Clinical monitoring is therefore recommended before starting treatment, during treatment and should be continued for some months after discontinuation of amiodarone treatment. Serum usTSH level should be measured when thyroid dysfunction is suspected.

Of the University of North Carolina-Chapel Hill is quantifying levels of neurosteroids in autopsied brains, and cerebrospinal fluid of patients with schizophrenia. Neurosteroids are hormones that have profound effects on the brain during neurodevelopment and adulthood in both men and women. It is thought that some antipsychotics act via altering levels of neurosteroids. Dr. Marx will measure levels of several neurosteroids, including allopregnanolone, dehydroepiandrosterone DHEA ; , and its derivative DHEAs, in brain specimens, and compare these levels between brains of patients with schizophrenia, bipolar disorder, depression, and controls. In addition neurosteroid levels will be measured in cerebrospinal fluid before and after treatment in patients with schizophrenia. This work may provide evidence that neurosteroids or related compounds have potential in the treatment of schizophrenia and hyzaar.

WARNINGS Hypotension Hypotension is the most common adverse effect seen with Cordarone I.V. In clinical trials, treatment-emergent, drug-related hypotension was reported as an adverse effect in 288 16% ; of 1836 patients treated with Cordarone I.V. Clinically significant hypotension during infusions was seen most often in the first several hours of treatment and was not dose related, but appeared to be related to the rate of infusion. Hypotension necessitating alterations in Cordarone I.V. therapy was reported in 3% of patients, with permanent discontinuation required in less than 2% of patients. Hypotension should be treated initially by slowing the infusion; additional standard therapy may be needed, including the following: vasopressor drugs, positive inotropic agents, and volume expansion. The initial rate of infusion should be monitored closely and should not exceed that prescribed in DOSAGE AND ADMINISTRATION. Bradycardia and AV Block Drug-related bradycardia occurred in 90 4.9% ; of 1836 patients in clinical trials while they were receiving Cordarone I.V. for life-threatening VT VF; it was not dose-related. Bradycardia should be treated by slowing the infusion rate or discontinuing Cordarone I.V. In some patients, inserting a pacemaker is required. Despite such measures, bradycardia was progressive and terminal in 1 patient during the controlled trials. Patients with a known predisposition to bradycardia or AV block should be treated with Cordarone I.V. in a setting where a temporary pacemaker is available. Liver Enzyme Elevations Elevations of blood hepatic enzyme values--alanine aminotransferase ALT ; , aspartate aminotransferase AST ; , and gamma-glutamyl transferase GGT ; --are seen commonly in patients with immediately life-threatening VT VF. Interpreting elevated AST activity can be difficult because the values may be elevated in patients who have had recent myocardial infarction, congestive heart failure, or multiple electrical defibrillations. Approximately 54% of patients receiving Cordarone I.V. in clinical studies had baseline liver enzyme elevations, and 13% had clinically significant elevations. In 81% of patients with both baseline and on-therapy data available, the liver enzyme elevations either improved during therapy or remained at baseline levels. Baseline abnormalities in hepatic enzymes are not a contraindication to treatment. Acute, centrolobular confluent hepatocellular necrosis leading to hepatic coma, acute renal failure, and death has been associated with the administration of Cordarone I.V. at a much higher loading dose concentration and much faster rate of infusion than recommended in DOSAGE AND ADMINISTRATION. Therefore, the initial concentration and rate of infusion should be monitored closely and should not exceed that prescribed in DOSAGE AND ADMINISTRATION see DOSAGE AND ADMINISTRATION.

Heroin is a synthetic opiate that is made from morphine. Both codeine methylmorphine ; and heroin diacetylmorphine ; are metabolized to morphine. Approximate detection times are listed in Table 11. The clearance half-life of opiates averages 5 hours. It takes 5 to 7 half-lives to clear 98% of a drug dose. Therefore, the presence of an opiate LOQ indicates exposure to that opiate within 2 to 3 days prior to specimen collection. Heroin diacetylmorphine ; is rarely detectable in body fluids. It has a half-life of a few minutes. Heroin undergoes rapid deacetylation to 6-monoacetylmorphine 6-MAM ; , which is about 6 times more potent than morphine. 6-MAM is further deacetylated to morphine; the effects of heroin are attributed to the combined effect of heroin, 6-MAM, and morphine. The presence of 6-MAM is conclusive evidence of prior heroin use. However, due to its short half-life, it is only detectable in urine for about 8 hours after administration. Codeine is also metabolized to morphine. The presence of morphine in urine can indicate exposure to morphine, heroin, or codeine. Illicit heroin often contains small amounts of codeine. The presence of both codeine and morphine in urine does not rule out the use of heroin; however, the ratio of morphine to codeine can be helpful in discriminating between heroin and codeine use. Ingestion of bakery products containing poppy seeds can also cause morphine to be excreted in urine. If excessively large amounts are consumed, this can result in urine morphine concentrations up to 2000 ng ml for a period of 6 to hours after ingestion. Due to first-pass metabolism, no pharmacologic effect is experienced from poppy seed ingestion. Following a dose of an opiate, the drug appears in the urine within 30 minutes. Peak urine output is reached at about 2 hours. The concentration then decreases at the rate noted above.
